If a minor is traveling to Croatia with you, provide the following documents:
Minor’s birth certificate.
Fill in a separate Visa Application for the minor , signed by both the parents.
Provide evidence from court if you are the sole custodian of the child.
Both the parents need to provide their passport and other identity cards.
If the minor is traveling alone, a consent form validated by the notary needs to be signed by both the parents. If parents are not present, a legal guardian can sign in place for them.
